What is DBT?

Dialectical behavior therapy is a structured treatment built by Marsha Linehan for people whose emotions arrive faster and harder than most, and who have learned to survive them in ways that cost too much. It pairs two things that sound opposed: radical acceptance of where you are right now, and relentless work on changing it. That is the dialectic. In practice it means learning concrete skills — how to get through an hour you cannot tolerate, how to say what you need without a fight, how to feel something without acting on it — and then practicing them until they hold under pressure.

Full DBT is a program, not a weekly conversation. It has four parts: individual therapy, a skills group that runs like a class, phone coaching between sessions for the moments the skills are actually for, and a consultation team that keeps the therapist steady. Plenty of people get real help from DBT-informed individual therapy instead, which uses the skills without the whole structure. Both are on this site, labeled for what they are, because the difference matters and most directories will not tell you.
